Schedule an Evaluation

Please call our office at 452-4517 to schedule an evaluation.

Before your child can be evaluated, a current prescription/referral from his/her pediatrician must be in place that requests specifically either Speech or Occupational Therapy Evaluation and Treatment.

If you are a Tricare client, then a current authorization must be in place before your child can be evaluated.

Please arrive at least 15 minutes prior to your child’s appointment, as there is a Registration Packet to fill out.
